SURGERY VIDEO STARTS AT 2:02. Warning: this may disturb some viewers. My nail gets cut in half and then ripped (gently) out of the nail bed with pliers. Yikes. So if you aren't into that kind of thing, then you may not want to watch. Backstory: I had been dealing with acute on chronic paronychia for over a year involving my thumbnail. Very frustrating (and painful!). I tried a partial removal (avulsion) of the nail plate in 2017, but after the nail regrew, I got repeated episodes of acute paronychia even worse than before. Oral antibiotics and a variety of different topical therapies and soaking regimens only helped a bit and only briefly; 6-8 weeks later, I would get another acute episode. So in 2018, I finally decided to visit my awesome dermatologist colleagues to have part of the nail avulsed again, but this time with destruction of the matrix to prevent that part of the nail from regrowing. Hopefully, this does the trick! UPDATE from 2021: The surgery worked out perfectly! Healing was quicker than expected and not very painful, much to my surprise. I have not had a recurrent episode of paronychia since then, and I am so thankful. At first, the nail didn't grow back (as we intended, which is why ablation of the matrix was performed. However, to my surprise, the nail has slowly expanded and filled in the space over the past 3 years since surgery. I suspect that the remaining nail matrix cells have slowly grown laterally and re-colonized the area where the matrix was ablated and destroyed. Basically, the roots of the nail have grown back in despite our efforts to kill them! The nail is almost back to its original size although there is still a small gap between the edge of the nail and the skin of the lateral nail fold. I'm a little worried that if it grows all the way back to the nail fold and then I will get a paronychia again. :-( But if that does happen, I'll go straight back to my dermatologist and ask them to do the same procedure all over again. It worked so well that I would happily do it again rather than have recurrent bouts of inflammation and infection.
